## Morpheus Outage Post-Mortem (April 11-12)
- 24-hour outage caused by OpenClaw read tool bug (GitHub #16717): Grok sends `path`, old handler expected `file_path`. Bootstrap failed silently.
- Fixed by upgrading to v2026.4.11: `sudo npm install -g openclaw@latest`
- Key mistake: added OpenAI provider to config but never changed `agents.defaults.model` — Morpheus was still running Grok the whole time
- Adner escalated to Claude on claude.ai → found fix in 20 minutes
- One-Hour Rule adopted: if no meaningful progress in 1 hour, stop, document, escalate to Adner
- Update Rule: always update ALL three boxes when upgrading OpenClaw
